After the teleportation, Raezhon was caught flatfooted by the rushing madman, only barely reacting before he launched into her.

Anyone who beats difficulty 3 can attack before the swarm.

In addition, Kaseeh can choose a major effect for the natural 20. An example would be starting out of their range, or giving himself an Asset to either his next attack or Defense check.

Repulsed, Kaseeh throws himself at the swarm with wide swings of his weapon.

I'll take the book's example as Kaseeh's asset, since plain damage from a broadsword feels ineffective against a swarm: You discharge a small pulse of power into your foe. Make an immediate attack against that foe (using the same stat as the action that caused the major effect). If the attack succeeds, it deals 4 points of electrical damage.
Maybe he jumps into action so hard, his systems overload and unleash a bit of excess electricity on the swarm.
