Y'breq's attack hits the creature, but fails to penetrate it's armor.

Y'breq does have other options: An attack roll of 17+ does extra damage. You can spend Effort on the attack to add extra damage (declared in advance, as usual). You can assist others, giving them an asset to their attacks.

A natural rolled 1 attracts a free GM intrusion. For this one, I'll say that the poison lasts two rounds instead of one, giving a hindrance to all actions for twice as long for Lyle.
