Crikey! Not sure which modifiers to use on that, so I'll just roll a straight d20 - could be modified by:

Leth turns his head and lets out a ghastly shriek in a voice quite unlike his own as Domarc charges him. He pivots away at the last second with a sudden, jerky motion, leaving Domarc stumbling past him and trying to catch his balance.

Zangua, go ahead with your Opportunity Attack.

The demon whips its tentacles at the demon hunter as he lunges in for another melee attack. Leth drops his bow, draws his short sword, and charges at Felor. The wraith reaches out with spidery black claws and glides swiftly towards Zangua...

Edit: The hunter ducks under the demon's flailing pseudopods and hacks a chunk out of its trunk with his axe. Zangua lurches out of the way of the wraith's approach. Leth's sword strikes Felor's armor and slides harmlessly off.

Felor and Mordred, you're up.

Seeing it pursue Zangua so intently, Mordred is sure he can sneak up on the wraith and drive his dagger into its shadowy form.

EDIT: That dagger damage should be +5 with proficiency.

Mordred sneaks up behind the wraith and thrusts his dagger into it, causing the wraith to writhe and twist like bedsheets tangled up in someone having fits.

Leth's blank, fixed expression does not change after Felor calls upon his deity.

"What's wrong with Leth?" Zangua manages to say in the midst of maneuvering his glaive for attack angles against the demon. Perhaps the question is rhetorical. He thrusts the blunt end of his polearm at the wraith to give himself some breathing room, and moves to the other side of the demon.

Using Mobile, I'm trying to get out of melee range against the wraith while still being in melee with the demon. If the wraith follows me, I'll use a Reaction with Polearm Master to make an Opportunity Attack when it comes into range (roll included below; ignore if not applicable).

Zangua cuts perpendicular slashes across the demon's eye with the blade of his glaive, then drives the butt end of it into the X he has marked. Yellow goo sprays and dribbles from the wounded orb.

The demon hunter continues his relentless onslaught...

Edit: ...he manages only a glancing slash with his dagger.

The wraith pursues Zangua as he circles around the demon. Zangua tries to swat it away with an opportunistic slash of his glaive. Some dark, vaporous fragments of the wraith disintegrate as the glaive passes through its body, but it keeps gliding inexorably closer...

Edit 2: The wraith's touch chills Zangua to the very marrow of his bones. Make a CON saving throw, Zangua.

The demon's eye swivels to fix upon Domarc. Make a WIS saving throw, Domarc.

With a dead-eyed, expressionless singularity of purpose, Leth swings his sword at Felor again.

Edit 3: Once again, Felor's armor absorbs the blow.
