This is the questionnaire to be answered on your character sheets, or in private note to me on this thread. The answers should definitely not be shared OOC with the other players, these are things for them to find out about you (or not) as the game progresses.
Make sure you respond as your character, I'm not prying into your real life answers to any of these. ;) However, I do want to know the answers even when your character does not know them, and just so we're clear, anything you don't explicitly tell me, I reserve the right to make up.
Phase 1: Family Information
1) How did your parents meet?
2) Are you their only child, or do you have siblings? If there are siblings, how many older or younger?
3) Who named you? Did they ever explain why they picked that name for you?
4) Have any of your family members died or left before the start of our story? If so, how did they die or why did they leave?
5) How did your family support you as a child? What kind of environment did you grow up in?
6) What's one big rumor that used to go around (positive or negative) about someone in your family? Was it true?
7) Is there anyone in your family that you particularly love or hate? Who?
Phase 2: Friendships and Favorites
1) Who (if anyone), aside from your family, was important to you in your earlier years? What sorts of activities and connections bound you to them?
2) What are your favorite and least favorite of the following?:
a) Season / Time of Day
b) Color
c) Food / Drink
d) Thing to Do
3) How did you start down the path towards your chosen class(es)? Who (if anyone) was involved in encouraging you down that path?
4) Provide brief story examples of how you began to learn proficiency in at least two of your chosen skills.
5) What is something you're deeply afraid of? What are two goals or experiences you're eagerly hopeful for?
Phase 3: Personality and Worldview
On the physical side we need skin color, hair color, eye color, height, weight, age, and whether or not there are any scars/birthmarks/tattoos on your body.
On the mental side we need left- or right-handedness (Ambidexterity is allowed only for Dex scores of 15+), sexuality and gender identity, any particular conditions you might be suffering from (IE dyslexia, color blindness, etc), whether or not you worship a deity, and of course Alignment.
Then, last round of questions, not *all* of these need to be answered but answer question 9 and at least three others:
1. What's one thing you wish you could go back in time two years and tell yourself?
2. Do you have a spouse or children of your own? Talk about them (or if not them, about a serious relationship and why it failed)
3. What is the thing about yourself that you're most proud of?
4. What is your most valued possession, and is there a story behind it?
5. What did you talk about in the last letter you sent by raven to another person? (Those of you who can't read or write, scribes are more than happy to take dictation for nominal fees)
6. What are the first two things you notice about strangers when you first encounter them?
7. Pick a political issue unique to D&D worlds (IE: Is mind reading magical ethical for law enforcement?) and discuss your take on it.
8. What are your views on the gods and their role in modern society?
9. What are three things on your 'bucket list' to do before you die?
