Fritz puts away his shield, pulls out his quarterstaff and follows Jhishma into the kitchen. "Cultists! Lay down your arms and surrender, or face the judgement of the gods!"

I think Nar's attack should have had no effect -- since his first swing didn't connect and kill, he doesn't get that bonus swing.

The first attack succeeded with a 12, and killed the cultist. The second one triggered, but missed with an 11.

"Fritz," says Jhishma, "lets take these bastards out to the Captain and his men." The dragonborn assumes Nar, Grok and Cindy are having an equal measure of success at the back of the house.

Fritz gives Jhishma a firm nod. "Yes, sir. And I must say, it's been an honor fighting at your side." He assists Jhishma in escorting the cultists to the guards.

The game pause is totally my fault - no excuses.
Fritz and Jhishma escort the cultists outside as the back door warriors clear the kitchen. A quick search of the main floor finds several more make-shift dormitories of the other rooms. After brief skirmishes, the cultists are cleared out of the main floor.

The dining room is piled with all kinds of gear - ropes, lanterns, oil kegs, crossbows, clubs, and a box of alchemists fire. Clearly they have shenanigans planned.

From upstairs, you hear a number of crashes, and bangs. You sense that the cultists upstairs are fortifying the stairway. There is also the stairs to the cellar.

"I'm on it!" says Nar as he bolts down the circular stairs to the cellar.

Nar charges down the stairs, the stench of fear greeting him as two crossbow bolts are released, one flying high, the other striking Nar's shoulder. (7 damage)

Nar bowls into the two of them, slamming them into the door behind them, and you quickly subdue them.

Behind the door, you hear a long scream, "No, no, NO,
NOOOOO-"
broken off suddenly by the sound of an axe striking a block.

Did that happen in the rooms on the first floor before the winding stairs, or at the bottom of the stairs? If the bottom, is the noise behind the secret door in room 11, the basement?

Right, there is a door at the bottom of the stairs into the basement (this wasn't described to you by the neighbors). The scream is from room 11.
