Heavy Undergrowth (dark green area): Costs 4 move squares, provides concealment (30%), +5 DC of Acrobatics, and +2 circumstance bonus to Stealth.
Shallow Bog: About 1 foot deep, costs 2 move squares, +2 DC Acrobatics and Stealth.
Deep Bog (blue wavy area): About 4 feet deep for Medium characters, Small must swim, +2 DC Stealth, provides cover to Medium or larger creatures (+4 AC, +2 Ref), provides improved cover to Small creatures (+8 AC, +4 Ref), Medium or larger can gain improved cover with move to crouch.
Roland and Weiryn are sickened (-2 to attack, weapon damage, saves, skills, ability checks).

Veroth pulls out an alchemists fire, and hurls it at the body of the center of the swarm, swooping overhead before turning back to the group.

There is no figure in the center of the swarm, it's just that the only picture ever made of this particular swarm showed it attacking someone. So basically you're throwing it at the intersection in the middle of the swarm.
Veroth misses with his alchemist fire, to his astonished surprise, so I'll roll for the direction to quicken things along. As for Darren, he peers into the bog, but fails to notice anything, cursing the fact that his vision is not as keen as Ujjaya's... The alchemist fire sails past the swarm crashing on the other bank and burning up harmlessly.

Durgrim is up next.

Durgrim moves up next to Darren, and readies his hammer against the oncoming mass.

Ready action to hit the swarm if it comes close enough. I'll roll it now so I don't have to later.

Note: I can channel energy if need be, but I'm guessing we have others that are better at the AOE game.

Weiryn steps forward as he chants, brushing past Darren to stand on the banks as orange light manifests between his hands. Throwing his right hand out, he releases a torrent of flame to wash over the swarm, ending it only when he closes his fist.

Weiryn casts burning hand on the swarm and burns the swarm badly, making several hundred minuscule bugs incinerate mid-flight. It would seem that fire, or at the very least area effect, are particularly useful against this cloud of buzzing undead bugs.

Ujjaya is up next.

Ujjaya doubts his arrows will do much good against a swarm of bugs. He focuses instead on the hulking shape underwater. What could it be? Does it pose any threat?

No, you are either delaying (making a full action after the shape in the water acts) or readying (make a standard action just before the shape in the water acts). Simply pick one of those two options.
Well, it is Ireena's turn then. Not really sure she can do much about the swarm, she will draw her crossbow and ready an attack against whatever is is Ujjaya spotted in the water.

A large creature made of tangled vines, moss, and leafs emerges from the bog, rivulets of brackish water dripping from its mass as it makes its way towards the party. Ireena (and possibly Ujjaya) can make a single attack on the creature before it acts before it ends up attacking either Darren or Weiryn (depending on what the dice gods decide).

Ireena's bolt flies wide of the hulking creature.

I'll go ahead and roll my dice before Jabes tells me if Ujjaya delayed or readied and I'll apply them once I get his answer.

Since Jabes is swamped (no pun intended), I'll go ahead and play his character. Right now, I'm a bit too tired to do this, so I'll get around to this this weekend, but I'll at least roll randomly as to whether Ujjaya readied or delayed as I can't really do any metagaming here...

Delay and ready are different things. When delaying, you delay your turn until some unspecified point in the future. You do not react to anything in particular (although you still could, but in which case, you'd get to react after whatever triggered you to take your turn), you simply specify at some point later in the initiative "I'll take my turn now", and then you get to perform all your actions.
When you ready, you declare that you are readying a specified action to react to a certain trigger (in this case, most likely the shadow in the water acting), allowing you to act right before the opponent who triggered your action.
Bot of those also result in your initiative changing.
Since you are shooting only one arrow before the plant creature acts, this is a readied action, yes.
Ujjaya shoots at the huge hulking form surging out of the water... but sadly misses. The creature will then strike Weiry for 11 points of damage, and also grapples him! Poor Weiryn is now dragged closer to the plant thing...

The swarm will then move towards to group, Durgrim swinging his mighty hammer at it... only to realize it has no effects. The multitude of undead bugs deftly dodge his huge clumsy hammer. Veroth, Durgrim, and Darren automatically take 5 damage points and 1 Con damage, they must also succeed at a Fort save.
