I will go with step of the wind and throw in my attack on the Myche eating slime. If I can save the inspiration die, I will.

With his father's battleaxe the half-orc monk cleaves savagely into the slime. The monster isn't looking too good.

Idea 1: Reach in, pull him out. understood that Urban will take the inevitable damage. Any rolls needed?
Idea 2: If he cannot be pulled free, Urban may be able to at least reach and touch him. If that is only option considering Lay on Hands to at least help him stay alive for another round. Again, damage to be endured.
Not an option this round but to answer your question, reaching in and pulling Myche out is a DC12 STR check. Spends your Action and suffer 3d6 acid. Like Baracus did for Lio.
As for reach in for Lay on Hands, sure yeah that can work. Next round.
Meta: On their turns this round (coming right up after your turn) the turtles are probably going to work together to try to free him.

Scooping up his torch and shield, Urban barrels down the tunnel, hoping to reach Myche in time...
With a battle-cry, the turtle-monks go to their brother's aid. Lio and Don-Yi both plunge their arms into the slime to pull Myche out, but strain as they might, he just can't get free. Rafiel, his kattari a-blur, viciously punches holes in the monster, which deflates, releasing its would-be meal.
The last slime, probably too stupid to know fear and driven only by a base instinct to feed, tries to overwhelm Baracus.

Urban is coming through like a freight train that hasn't even been invented yet, but he can picture it in his mind, chugging down the track with a big plume of smoke billowing out of the top. It's unstoppable.

I'd argue, even without seeing the DC prior, rolling a twenty has little cause to use the inspiration.
Moving away from the slime without disengaging, hoping to pull it's OA.

Erden's fiery bolt lights up the tunnel as it streaks towards one of the new slimes. It hits center mass, causing the monster's flesh to melt and slough off in disgusting globs. (9 fire damage)
Urban's shield slams into the same slime twice with a flash of holy light. (11 bludgeoning + 13 radiant). It is pushed back by the fury of Urban's attack, but then both ochre slimes retaliate, striking at the paladin with their disgusting appendages. (17 bludgeoning, 8 acid)
Ybus seeing Urban in trouble, sends a beam of crackling energy streaking down the tunnel. (7 damage)
Baracus whales on the remaining green slime with his father's axe, nearly chopping the thing in half. (12 slashing)
Rydler steps in and strikes with surgical precision (11 damage) then steps away to draw the monster away from the beleaguered turtles. The monster takes the bait, its appendages flailing at both Rydler and Baracus, but it's grievously injured and its attacks are ineffective. (both attacks miss)
The turtles help Myche stumble away to relative safety...except for Rafiel, who doesn't let up for one second, and the slime at the channel crossing finally succumbs to its injuries.
Now there are only the two smaller ochre ones in the tunnel, but Urban is in big trouble. Round 4 ends. Begin Round 5. All PC's are up in any order.
