Depends on checks.
There's a door and you state you try the knob, if it makes sense you have the skillset to open an unlocked door it happens no roll. If it's something locked a skullduggery type roll to lockpick or bumping the door with a shoulder might be suggested by the PC if they think that's more in character.
Another example would be a high chain fence how do you get around, you could try and just climb up, I'd state an athletic check and give difficulty, if you're trying to get some way up the wall to jump off to climb up and over coordination. Knocking on the back door to a business and see if they don't mind you coming through to get around a social check. You might ask if there's a dumpster on wheels close by you can push over, if not would you like to spend a story point to make it so comes to mind as the GM to ask the player.
I figure once per session although not static in relation would reset each month. Although I hope we can get 'a session' done in a month I get life and work might slow things down.
