Now that you saw the rats:
The extremely vicious Barovian rat provided an ideal host for teh supernatural infection. The rats prefer avoiding the streets and instead move from house to house through cracks and crannies too small for zombies—though they are bigger than regular rats, and must often force holes large enough to squeeze through.

Conditions and Modifiers
Roland and Kayleigh are sickened (-2 penalty on attack, damage, saves, skill checks, and ability checks)
Roland is paralyzed (Dex and Str of 0, helpless, can take purely mental actions, can move through space but costs s squares)
Some zombies and zombie rats are panicked for 1 minute (flee at top speed from source of fear as well as other danger encountered along random path, can,t take any actions, -2 on saves, skills, and ability checks, if cornered cowers)

I'll play Kayleigh right away, unless Roland can take a purely mental action?.

Kayleigh is going to attack the unaffected rat next to her, and if she kills it, she'll try to get in position to try and help Roland next round.

Kayleigh pulverizes the rat.

The maggots are up next. Since they only have Roland as a target, they'll keep attacking him.

Yes, unless you miss your Fortitude saves against these attacks.
Roland is hit twice for 7 points of damage total (2, 5). He needs to make 2 more Fortitude saves.

Veroth five four steps into the corner and stabs the beasty to his (map) north. His rapier bursts from his shadowy folds and attempts to pin the creature like a collected butterfly.

Not sure if flanking. If flanking, then add +2 to hit on roll, and add sneak attack damage, which I rolled just in case.

There is no flank in this situation, however, since that rat was panicked and had nowhere to flee to, it is most likely cowering right now, so it loses its Dex bonus to AC, which allows for the sneak damage to apply.
Veroth thrusts his rapier into the rat, skewering it. The rat becomes inert as its limp body slides off Veroth's blade.

Darren steps forward to get a better angle of attack and makes a sweeping strike with his warhammer on the zombies in front of him (D5 and D6).

An attack of opportunity? From what? You made a 5-foot step and then attacked. And even if you meant the zombie in d5, it's panicked and does not attack anymore...
Darren takes a might swing of his hammer and smashed two zombies. They are still standing, however, thanks to their natural resilience.

It is the zombies who are up next. Two of them will be fleeing, one of them will attack Darren.

The zombies miss, so now it is the zombie rats' turn. and they will both flee.

Random direction of first zombie fleeing (once it reaches the alleys behind the houses) - (1d4)

Nope, nothing official, but d20pfsrd.com has a couple of cool crit and fumble generators. They usually advise that you have players "confirm" their fumbles before using them

You make an attack roll using the same modifiers (just as you do on a critical hit confirmation). If the confirmation roll would be a miss against the opponent's AC, then you've confirmed the fumble and must take the penalty.
