The whole away team made it safely across the ravine safely. Now there is only jungle between you and the outpost. You can barely make it out through the dense foliage, a mere kilometer away.

Making it throught the jungle here is an extended task. There are dangers here, poisenous plants for example. You may evaluate the plants (Reason+Science or Empathy+Medicine), use basic survival skills to get through (Control+Security) or anything else you can come up with that will fit the story. (Since this is a pbp, I'll assume you come up with stuff that makes sense, if things get out of hand we'll do things differently next time.)

This extended task has a Progress Track of 16, a Magnitude of 3 and a basic Difficulty of 4 for Tasks to overcome it. Since you are trying to maintain a decent pace, you may only attempt a maximum of three Tasks (total) to overcome this Obstacle. For each successive Task attemted by the same character the Obstacle increases in diffuculty by +1.
Relevant info on Extended Tasks:

Whenever a Task is completed towards an Extended Task, that character rolls two [CD], plus a number
of additional [CD] equal to Skill used for that Task. The total rolled is the number of points of Progress
marked off. If the total is five or more, then there is also a Breakthrough made on that Extended Task.

As I read this: The goal is to get to get 3 Breakthroughs, and breakthroughs are scored by either getting 5 or more progress on a Task, or finishing 16 on the progress track. Note that the difficuly goes down for each breakthrough.

Following Mr. Sevek's instructions to avoid harmful flora & phaser in hand in case he encounters the dangerous fauna, Thyran charts a course to take them in the direction of the outpost, ensuring that they stay on course and don't wander off. Although from an icy homeworld, he is quite familiar with looking for the moss on the north side of a tree, so uses his nature and survival training to guide the crew.

I should have waited for Sevek to roll his 7CDs to see if he made a breakthrough, reducing difficulty to 3.
5 success, 8 points of progress, so used one momentum, but added either 1 or 2 back into the pool
Breakthrough because 5+ points of progress rolled, so Difficulty is reduced by 1

by my count, Sevek got 4 points of progress, so no breakthrough, and Thyran got 8 points for one breakthrough. We now have a total of 14/16 points. I'm counting CD as 0 for (3,4), 1 for (1,5,6), 2 for 2. Not sure if "effects" are in effect, but Sevek and Thyran each triggered 2.

The next person has an opportunity to complete the extended task with a good CD roll, 5+ & beat the 16 total.

Quite right. 6 Successes, whole of 2 Breakthroughs, and You make it! +2 momentum, and deducting the one you used, the party now has 3 momentum!
The away team works together and identify several new species of poisonous plants, some which even send out poisonous spores that would have been devastating for any less talented and determined group of individuals. Logging the finds for submittal to the Starfleet Archives, you proceed safely to the outpost.

The science station is intact, but heavy damage is evident along the structure’s exterior: mostly dents and punctures mar the surface, but a few areas of phaser fire are evident as well.

Inside the outpost, the place has been trashed. From the living quarters to the kitchen to the laboratories, it is obvious someone or something wrecked the place without regard. All communication devices have been smashed.

As you are ready to give up on your search for life, you find one last storage area with the door closed and locked. You hear a voice from inside.

On a cot at the back of the storage area sits a middle-aged human woman. She cradles in her arms a human male. It is hard to tell his age because of his strange appearance. His sweat-soaked face is covered with hair, and even as they watch, the ridges of his eyebrows twitch and pulse, as if changing in sporadic fits.

When you enter, the woman looks up at you, her eyes unfocused and filled with tears. "Thank you for coming, but It’s too late. They are all gone, and Jasper is..." She cannot finish her thought through her sobbing.

Nadim blunders in, medical equipment veritably popping out of his bag into his hands with a mixture of excitement and concern. "Jasper is this man here?" he asks as he begins to take readings and look at how to stabilize the patient. Although tempted to provide a sedative, his training reminds him that he should understand the patient's condition before he starts jabbing him with a hypo spray.

Assuming this is Empathy + Medicine (11 + 4 = 15). Note that Emergency Medicine is a focus -- so I'm tempted to try and suggest it qualifies here.

Nadim takes out his field medical kit and gets to work. Dr. Jasper seems to have been beaten severely, seemingly by the primitive humanoids you encountered earlier, judging by the wounds (blunt trauma and some shallow jagged puncture wounds). He is in need of immediate surgery, so Nadim takes out a field surgery kit and extracts debris from the wounds. After about half an hour of intense surgery Nadim is able to stabilize the patient. Dr. Shipp is watching the proceedings in a near shock-like state.
